In order for the specialist surgeon or dentist to provide you and your patients with the best possible treatment course, we ask that you provide the following information for all new patient consultations:
Letter of referral including your current treatment plan for the patient and details of your request for treatment. Please contact our office if you would prefer to use our printed referral pads, these can be sent to your office today.
A current OPG (within 12 months) or periapical radiograph of the area of concern. The OPG must be labelled with the patient’s full name and date of birth.
